Case 6: The Sleeper
History: Lulu is a 6-year-old female spayed black Pomeranian…
Good thoughts on Lulu. One person suggested she may have an endocrine disease; in a dog with a fairly normal exam when it comes to the heart, lungs, and abdomen, searching for an endocrine cause of lethargy and poor hair coat is a good idea. Another person suggested the heart, which could be a good cause of lethargy based on history alone; however, the physical exam doesn’t really support that. Another suggested doing a fecal test to check for parasites. Good idea, since her hair coat is poor (suggesting her nutrition may not be well absorbed) and she has had some diarrhea.
There were some good ideas after the most recent update on Lulu’s case. One person noted the elevated amylase and lipase, which can be consistent with pancreatitis; a CPLi test to test for pancreatitis was run. Two people were concerned by the low blood glucose and wondered about diabetes. Diabetes typically results in increased appetite, increased drinking, increased urination, and weight loss. Lulu didn’t have any of these clinical signs. While it is possible for diabetes to be discovered incidentally, it usually causes clinical signs by the time it’s causing either severe hyperglycemia or severe hypoglycemia. So diabetes wasn’t as high on my radar.
However, I was very concerned about the prospect of an insulinoma.
Insulinomas are tumors of the pancreas that cause hyperproduction of insulin. Increased insulin decreases blood sugar; and signs may be more severe after meals, which fit with the owner’s description of an energetic dog who got sleepy after eating. They can be tricky to diagnose, as the dog may have normal blood sugar much of the time; the best way to diagnose them is to test an insulin level at the same time that the blood glucose is low.
Also, don’t forget the raging urinary tract infection seen on the urinalysis!
